
When registering a child, parents are to bring the following items:
Birth certificate
Dates of immunization; and any pertinent information concerning the child’s health
Proof of Residency (electric/utility bill)
Custody documentation (if applicable)
Foster care documentation (if applicable)
Enrollment eligibility:
A child is eligible for Kindergarten if he/she has attained the age of 5 on or before the first day of September. A child who is 4 years and 7 months to 5 years on or before the first day of September may be eligible if he/she passes the Kindergarten screening and receives Administrative approval.
The Department of Health, with support from the Pennsylvania Department of Education, requires the following immunizations for students entering our schools:
Three (3) doses of Hepatitis B vaccine
Two (2) doses of Measles, Mumps, Rubella (MMR)vaccine
(1st vaccine must be given after 1st birthday)
Four (4) doses of Tetanus and Diphtheria (TD) vaccine
(5th dose if fourth dose is before fourth birthday)
Two (2)doses of Varicella (Chicken Pox) vaccine or a note
from parent with month/date of disease.
Three (3) doses of Polio Vaccine
Child must have a minimum of 1 dose of each vaccine to begin school.
